After we have an initial conversation, we can set up an appointment for evaluation and assessment. Teletherapy To set up your telehealth session, I will send you a welcome email after our initial phone conversation. From there, you will be able to create a portal for my practice with a user name and password that will allow you to complete required forms and join the session at the appointed time. At the time of our session, just log in and click on the “Join Telehealth” or “I’m ready” button. If you have trouble logging in, please call me at 262-221-3971. Insurance I am in network with several insurance plans. Please contact me for an updated list of insurance providers I am contracted with. I can also work with your insurance out of network if I am not on your plan. I bill insurance directly and electronically on your behalf. I can help you understand how your insurance plan works for my services, and I am experienced in helping you make the best use of your insurance. I recommend that you also call your insurance company before starting therapy to understand your deductible and copay amounts. Copays are due at the time of the session. Fees Outside of insurance, my fee is $180 per 50 minute session for couples therapy and $150 per 50 minute session for individuals. I accept cash, checks, credit cards, and HSA or FSA cards. Payment is due at the time of the session. A statement or receipt is available upon request. In some cases, based on financial need or out of network insurance, the fee can be negotiated. I am happy to discuss this with you at the beginning of our work together, or if your circumstances change. Client Forms Please fill out the required paperwork on the portal before the session using the instructions above.
